First Visit

Please bring the completed and signed health history sheet, available on the website or sent in the mail with the information packet. If your dentist has provided a referral slip or X-rays to you, please bring those with you. Lastly, if you will be using insurance coverage, please bring a completed and signed dental insurance claim form or a copy of your dental insurance card.

At your first visit, you will be greeted and accompanied to our consultation and examination room, where you will meet Dr. Brochu who will complete a comprehensive oral exam. While documenting the case, Dr. Brochu will share his findings with you and provide recommendations for treatment. A quote of fees for treatment is included during the discussion. At this point, we encourage you to ask any questions that you may have so you can have as much information as possible about the process and benefits of treatment. Following the exam, you can meet with the office manager to discuss payment options and insurance questions. If the decision is made to start treatment, an appointment is scheduled for diagnostic records. (See Appointments: What to Expect")
